Columbia Heights Seniors Transitional Care
Minnesota Transitional Care is for patients who have been discharged from the hospital but still require short-term rehab services and special care in order to make the transition from hospital to home.

Skilled nursing services, a therapeutic recreation/life enrichment program and three meals a day are included. There is a specialized transitional care/short term stay area. There is an area that provides specialized care for people with Alzheimer's and dementia. All faiths are welcome.
